Manage Your Health Care Online With MySite

MySite at Beth Israel Deaconess Hospital-Milton is a secure online resource that gives you free instant access to your health information, including laboratory and radiology results, medications, allergies, conditions, visit history, discharge/aftercare information, and more.

You can also submit updates to your personal data, including emergency contacts. In addition to handling your own care, parents, representatives and health care proxies with authorized consent, will also have access to records. If you would like to grant a representative access to your Personal Health Information (PHI) contained in your MySite portal, please download and complete the Authorization for Access to PHI via MySite Form and return to the Medical Records Department at BID-Milton.  The form must be returned to Beth Israel Deaconess Hospital-Milton with original signatures in order for the request to be accepted.

If you would like to request a copy of your medical records, including information that may not be accessed through the MySite Portal, please download and complete the Authorization for Disclosure of Privileged Information Form, and return to the Medical Records Department at BID-Milton at the address listed above.
